How To Register Your Dog for Group Training Classes
Creating a Better World for Pets and People Since 1993
PLEASE NOTE! Group classes are NOT suitable for aggressive dogs. Private sessions are needed to effectively rehabilitate these dogs. If you have any questions about the suitability of your dog for group classes, please call our office to speak to our behavior consultant, Diane Chandler CPDT-KA.
You must pre-register your dog by mail or at the school before the start of your course. To register by mail, call our office at 630-232-0492 or e-mail us at clixworld@aol.com to have the registration forms sent or faxed to you. Remember to give us your name and complete address. Registrations will be honored on a first come, first serve basis. Registrations will only be valid if accompanied by full payment of the course fee. If your payment is received after the course has filled, it will be refunded to you. There will be no refunds after the course begins. Payment is by check or cash only. We do not take credit cards at this time. There will be a $40.00 fee for any NSF returned checks.
Health Form A health form will be sent or faxed to you . Please have your veterinarian fill out and sign the health form after examining your dog. ALL vaccinations appropriate to your dog's age must be current, and a fecal tested negative within 2 weeks of the start of class. This is for the protection of YOUR dog. The form may be faxed to 630-365-5310 or bring it to your dog's first class. NO DOG WILL BE ALLOWED IN CLASS WITHOUT THE HEALTH FORM!
